This RN Case Manager Position Is 100% Remote And Candidates Can Apply In Any State But Must Work The Listed Hours.

Licensed Nurse Practitioners Are Preferred, But Not Required. Normal Business Hours: Monday - Friday Between 10:30 AM - 7:00 PM EST For A Scheduled 8 Hour Shift.

Weekends And Holidays May Be Required Depending On Department Needs. Currently, Weekends And Holidays Are Invite-Only And Covered Voluntarily.

Less Than 5% Travel May Be Required For Site Meetings, Training, Or System Updates, Or To Fulfill Licensing Requirements, Such As Fingerprinting.

The RN Case Manager Is Responsible For Evaluating, Planning, Implementing, And Coordinating Over The Phone And/or In Person All Case Management Services And Team Members To Assess The Member's Health Needs To Support His Or Her Overall Well-Being.

RN Case Manager:

  • Collaborate To Create A Proactive Action Plan To Address Issues To Improve Short And Long-Term Outcomes And Opportunities To Improve The Overall Well-Being Of The Team Through Collaboration.
  • Through The Use Of Clinical Tools And The Analysis Of Information / Data (Such As Surveys, Statements, etc.....), Conduct An Analysis Of The Needs And Benefits Of The Participants About The Programs Offered By The Plan Sponsors.
  • Applying Clinical Judgment To Reduce Risk, Solve Serious Health Problems And Social Indicators.
  • Uses Case Management Procedures according to the Company's Laws And Regulations.
  • Uses Interview Skills To Ensure The Participation Of The Main Group And Determine Their Health Status And Health Needs Based On Key Questions And Discussions.

Qualifications Required:

  • Must Have A Valid RN License, No Restrictions On State Of Residence. If They Are Within The Jurisdiction Of The Compact Nurse Licensure, The Right To Use The Compact License Is Unrestricted.
  • Willingness And Ability To Obtain Another State License For Work (Company Paid)
  • More Than 3 Years Of Clinical Experience In Health Care As A Registered Nurse (General Medicine, Post-Surgery, Specialties Including Pediatrics, Intensive Care, Case Management, And Planning)
  • Ability To Use Multi-Screen Computers, Multitask, And Sit For Long periods.
  • Dedicated Personal Workspace, Free Of Distractions And High-Speed Internet

Candidate Qualifications:

  • Compact Nursing License
  • 1+ Year Experience In Case Management
  • Certified Material Manager (CCM).
  • Strong Customer Service Skills Including Customer Focus, Understanding Of Issues, And Problem Identification/Resolution.
  • Experience With All Forms Of Microsoft Office Including PowerPoint, Excel, And Word
  • Ability To Communicate On The Phone
  • 1+ Year Of Experience Using A Computer And Navigating Multiple Monitors While Communicating On The Phone.
